Information shared by you in session will be kept confidential.

 

Information will not be released without your written consent, except for professional consultation if needed and unless required by law.

 

I am required by law to disclose information pertaining to suspected child abuse, the inability to care for one’s basic needs for food, clothing or shelter, and threatened harm to oneself or others.

 

One person per telephone session unless other arrangements have been agreed upon. Taping of sessions is prohibited without our consent.

 

Appointment and fees

 

A $25 nonrefundable scheduling fee is required to be paid in full to schedule your appointment. Your appointment will not be scheduled or secured until after this fee is paid. This fee will be deducted from your remaining balance for your session. Full Payment is expected before the session begins. All appointments, not cancelled within 24 hours in advance of session, will be billed to the client for the full amount.

 

Our counseling rates are as follows: Individual, $100 (45 minute session) $125 (60 minute session), Couples and family, $125 (45 minute session),$150 (60 minute session).  If extra time is needed additional rates will apply. Sliding Scale slots may be available however clients must meet low income requirements.  Payments are accepted through: Credit card, cash, insurances, out of network, pre-authorizations and single case agreements.
